New Graduates Are Welocomed SUMMARY: Provides professional nursing care and services to agencys clients according to Physicians orders, nursing standards of practice and the agencys policy and procedures. DUTIES AND RESPONSIBILITIES:Provides care and services according to scope and standards of Nursing Practice and adheres to the Code of Ethics for Nurses.Respects client and employee rights and privacy, ensures security of protected information, practices in an ethical manner and is compliant to the agencys compliance program and privacy policy.Follows agencys policy and procedure regarding infection control measures through proper hand washing, bag technique, use of Personal Protective Equipment, principles of aseptic technique, exposure control/reporting, care of equipment, safe needle technique and other protocols consistent with agencys infection control program.Assesses and regularly reassesses the clients needs and his/her response to care, treatment and education.Develops implements, evaluates, revises, and provides input to the clients plan of care to include the physicians plan of treatment, problem list, nursing care plan and the aides plan of care. Provides care according to the plan of care. Completes all client documentation, to include written records, reports and notes in accordance with agency policy and procedure following client visits or contacts in an accurate, timely, complete, concise and legible manner.Actively coordinates the care of the client through communication and written documentation to the physician, case manager, and supervisor, other disciplines involved in the clients care, labs, equipment, or supplies, and community resources.Supervises ancillary staff involved in the clients plan of care. Provides education to clients and families through assessment of the learning needs, development of teaching objectives and plan, implementation of the teaching plan, evaluation and revision of the teaching plan as needed.Provides nursing services, treatment, and diagnostic, preventive and rehabilitative procedure as prescribed by the clients physician and in accordance with the agencys policy and procedure. Assists with the orientation, and/or guidance of new staff members (professional and non-professional), students, volunteers, and others.Complies with agency policy and procedure and provides care in according to federal, state, and local laws.Participates in agency and community programs.Participates in in-service education programs of the agency.Interprets philosophy and objectives of the agency to clients, families and others.Participates in the agencys Performance Improvement Program.Performs other job related duties as may be required or directed by the supervisor.SPECIALIZED SKILLS AND COMPETENCIESDemonstrates knowledge and clinical expertise within the scope of practice of a Registered Professional Nurse practicing in New York State.Excellent communication skills.Excellent organizational ability.Must be computer literate, including ability to learn and use program-specific software.Ability to direct home care workers.Demonstrates knowledge of New York State Dept of Health regulations for Licensed Home Care Service Agencies.PHYSICAL REQUIREMENTS:Will spend varied percentage of working time indoors or outdoors depending on program and/or client needs. Will encounter prolonged standing, walking, frequent lifting, positioning clients, pushing and/or transferring clients; lifting supplies and equipment; frequently requiring twists, stoops, bends, squats, and occasionally climbs stairs.WORKING CONDITIONS:Office setting and regular home visits, including attendance at meetings. Considerable exposure to stress. Use of auto or public transportation, walking and stair climbing. May be exposed to client elements and illnesses as well as a wide variety of circumstances and conditions within the home setting. Evening and weekend work required as necessary to attend meetings, meet deadlines, and/or as part of on-call requirements.
